This was a project that would demonstrate how persons who use Home and CommunityBased Care (HCBC) could be meaningfully involved in determing what these services should be and how they should be provided in their communities. The project had six goals: (1) to expand the awareness of the local/state HCBC system; (2) to involve as many people, specifically senior consumers, in the process of deciding the direction and priorities of HCBC; (3) to educate the community about the role that the HCBC plays in the infrastructure of a community and the consequences of a system failure; (4) to develop new local leadership and support for HCBC; (5) to provide consumer-driven recommendations for area and state plans for HCBC services; (6) to demonstrate a replicable model for HCBC Community Decision Making in other states, with manual and video production.

The United States has been at war longer than any time in its history. While thousands have been wounded in these conflicts, advances in battlefield medicine mean many of our troops survive catastrophic wounds. The nature of many of their wounds, however, means some require long-term caregiving support. The Elizabeth Dole Foundation commissioned RAND Corp to assess the needs of military caregivers, scan the services available to them, and identify how their needs are -- and are not -- being met. This report reviews existing research on the needs of caregivers in general, and assesses how lessons learned can be applied to military caregivers. We also present information gleaned from military caregivers themselves and from policymakers and program officials who either directly support, or advocate on behalf of, military caregivers. We provide a snapshot of the number and characteristics of military caregivers, the roles they serve, the physical and emotional impact caregiving has on their lives, and the resources available to them. Military caregivers tend to be younger women with dependent-age children, dealing with a different set of patient variables than the general caregiver population. Along with typical caregiver responsibilities, military caregivers also act as case managers navigating multiple health systems, advocates for new treatment, and financial and legal representatives. Many are also raising children and holding jobs outside the home. Studies indicate that caregivers in general suffer from physical strain and overall worse health and tend to put their own concerns behind those of the individuals for whom they are caring. Military caregivers suffer disproportionately from mental health problems and emotional distress. Many government programs are still in their infancy, and community resources are scattered and uncoordinated. Difficulties are presented by differing eligibility criteria, lack of access, and the way caregivers' needs change over time.

The second National Survey of Child and Adolescent Well-Being (NSCAW II) is a longitudinal study intended to answer a range of fundamental questions about the functioning, service needs, and service use of children who come in contact with the child welfare system. The study is sponsored by the Office of Planning, Research and Evaluation, Administration for Children and Families (ACF), U.S. Department of Health and Human Services (DHHS). It examines the well-being of children involved with child welfare agencies; captures information about the abuse or neglect that brought the child into the study; collects information about the childs family provides information about child welfare interventions and other services; and describes key characteristics of child development. Of particular interest to the study are childrens health, mental health, and developmental risks, especially for those children who experienced the most severe abuse and exposure to violence. The study includes 5,873 children ranging from birth to 17.5 years old at the time of sampling. Children were sampled from child welfare investigations closed between February 2008 and April 2009 in 83 counties nationwide.

A series of questions on the need for personal assistance was included in the sixth interview of the 1985 panel of the Survey of Income and Program Participation (SIPP) and the third interview of the 1986 SIPP panel, which were both conducted in September through December of 1986. These questions collected information on whether persons 15 years of age or older required the help of another person to (1) take care of personal needs such as dressing, eating, or personal hygiene, (2) get around outside the household, (3) do light housework such as washing dishes, straightening up, or light cleaning, (4) prepare meals, and (5) keep track of bills and/or money. The universe for SIPP is the non-institutional population. This survey does not include persons living in nursing homes or other institutions. Persons were identified as needing assistance if they reported that they received personal assistance with one or more of the five activities listed above because of a health condition that had lasted or was expected to last three months or longer. The same survey also collected information on persons who provided care services to others. The questions on the need for care and on the provision of care are reproduced in appendix E. This report examines the characteristics of both persons who needed assistance and persons who provided care.
